Facility Evaluation Report
On 8/22/2025 at 1:20PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Post-Licensing inspection. LPA met with Program Director, Christopher Yarruhs and informed him the reason for the visit.
LPA toured the facility including but not limited to activity rooms, resting room, common areas, bathrooms, and kitchen. Facility does not have an outdoor activity space. Smoke detectors are interconnected with sprinkler system. Carbon monoxide detectors were observed. Fire extinguishers were observed to be full and last serviced on 10/21/2024. Clients bring lunch from home and facility can provide lunches for clients if needed. Facility has snacks and food supplies available for clients. Hot water temperature in the bathroom was measured at 108.2 degrees F. All observed toilets and hand washing stations are maintained in a safe, sanitary, and operating condition. First aid kit was complete. The last disaster drill was conducted on 8/19/2025.
LPA reviewed 1 client and 3 staff files starting at 1:35PM. Day Program currently does not have clients that need assistance with medication administration.
No deficiencies are being cited on this date.
