Facility Evaluation Report
On 5/6/2025 at 9:55AM, Licensing Program Analyst (LPA) G. Luk conducted a Pre-licensing Inspection. LPA met with Licensee/Applicant, Elinore Ramas and Program Director, Christopher Yarruhs.
LPA toured facility including but not limited to offices, wellness room, clinic, common areas, bathrooms, and kitchen. Facility does not have an outdoor area. Facility's refrigerator temperature was recorded as 36 degrees F. Facility will purchase food prior to admitting clients to the day program. Hot water was measured at 108.8 degrees F in one of the bathroom sink. Facility purchased furniture for the wellness room and will set up furniture prior to admitting clients to the day program. Carbon monoxide detectors were observed in operating condition. Smoke detectors are interconnected with the sprinkler system. Fire extinguishers were observed to be full and last serviced on 12/6/2024. First aid kit was complete.
No issues noted during inspection. LPA observed that facility is ready to be licensed. This report will be submitted to the Centralized Application Bureau (CAB) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be required.
