On 9/29/2023 at 1:45 PM, Licensing Program Analyst (LPA) Greg Clark arrived unannounced to conduct a case management visit. LPA met with Yuri Flores, Director of Human Resources (DHR) and explained the purpose of the visit.
LPA went to the facility to deliver an Immediate Exclusion letter. Immediate Exclusion letter was delivered to DHR. LPA has advised DHR to disassociate the individual from their facility. DHR stated that S1 is currently not a staff member at this facility.
Exit interview conducted and a copy of this report provided.
