Facility Evaluation Report

On 3/26/2026 at 1:45 pm, Licensing Program Analyst (LPA) Y.Brown conducted an unannounced annual required inspection. LPA met with Program Manager Marrione Henry and explained the purpose of the visit. Marrione phoned the Administrator (ADM) Allen Henry and the ADM arrived shortly after.

LPA toured the facility including but not limited to activity rooms, bathrooms, kitchen, common area, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 106.1 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition.

The supply of extra hygiene was available for residents. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 06/18/2025. The First aid kit was observed to be complete. LPA reviewed five (5) staff records and five (5) client records. During record review LPA observed that The Emergency Disaster Plan was last reviewed on 3/14/2025.
