Facility Evaluation Report
On 1/8/2026 at 9:45AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Required - 1 Year inspection. LPA met with Program Coordinator , Jackie Bailey and explained the purpose of the visit.
LPA toured the facility including but not limited to activity rooms, sensory room, common areas, bathrooms, kitchen, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguishers were observed to be full and last serviced on 7/29/2025. Clients bring lunches from home or residential facilities. However, facility has some snacks available for clients. Hot water temperature in the hallway bathroom was measured at 113.8 degrees F. First aid kit was complete. Last emergency drill was conducted on 12/12/2025. LPA reviewed 3 clients and 3 staff files starting at 10:10AM.
At 10:45AM, LPA observed clients (C1, C2, C3) did not have medical assessment on file during record review.
At 11:00AM, LPA observed client's files were incomplete.
The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiencies may result in civil penalties.
