Facility Evaluation Report

Licensing Program Analyst (LPA) G. Luk made an unannounced visit on this day for the purpose of conducting a Case Management, Health and Safety visit. LPA met with administrator, Lisa Evans and explained the purpose of the visit.

Present at time of visit were 0 clients and administrator provided access to the facility. LPA inspected the interior and exterior of the facility. LPA verified that the facility had running water and electricity. LPA tested water temperature in client's bathroom which measured 119 degrees F. LPA inspected food supplies and observed 2 days worth of perishable and 7 days worth of non-perishable foods.

LPA was unable to interview any staff or clients at the facility during visit. All clients and staff were on outing during visit.
