Facility Evaluation Report

While conducting pre-licensing inspection for change in ownership for this facility, Licensing Program Analyst (LPA) Delmundo observed the following:

-at 12:30 pm, lavatory in the ensuite bathroom not draining properly.

-at 12:37 pm, drawer handle in one of the residents' rooms broken.

-at 12:40 pm, fireplace cover not secured.

-at 12:43 pm, outside wall planks of the building in back and side yard side were coming off and beginning to rot.

Deficiency is cited from Title 22 California Code of Regulations, and listed on 809D. A $250.00 civil penalty is assessed for repeat violation within 12 month period of section # 80087(a). This section was cited on 2/28/25. Failure to submit proof of corrections by plan of correction due date may result in additional civil penalty.

Deficiency and plan and proof of corrections were discussed with Beth Nunez and Jordan Nunez..
