Facility Evaluation Report
At 10:15 am on this day, October 15, 2025, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with staff, Aileen Fajardo and Estelita Tubangui. LPA called and left message on Sheilha Muniz's, administrator (ADM), voicemail. Joseph Tabuza, licensee, arrived at around 11:20 am. ADM arrived at around 11:40 am. Licensee left at around 1:00 pm.
LPA toured the facility inside out with Aileen Fajardo. LPA inspected the kitchen, dining and activity areas, bathrooms, residents' rooms, laundry room, side yard and backyard. Food were checked and observed supplies of 2 days of perishables and 7 days of non-perishables. Central storage for medications was observed locked.
Facility has 2 in 1 smoke and carbon monoxide detector that was tested and observed in operating condition. Hot water temperature in one of the bathrooms was tested and measured at 114.4 degrees Fahrenheit. Facility conducts fire and earthquake drills every quarter, and records showed last conducted October 1, 2025 and October 2, 2025 respectively.
LPA reviewed 5 staff and 5 residents files, and interviewed 1 resident. Medications checked, and compared with doctor's orders and LIC622 Centrally Stored Medication and Destruction Records. Residents' P& I were checked and compared with last recorded balance.
