Facility Evaluation Report

While at the facility for other reason, Executive Director (ED) Joseph Villanueva conferred with Licensing Program Analyst (LPA) Delmundo. ED stated that in response to the facility citation (deficiency issued on May 26, 2026) on having scissors in the art studio, they have implemented twice per day checks of the art studio drawers & cabinets. ED also stated that residents bring in their own items, and that he cannot think of a solution to monitor the art studio 12 hours per day for each resident who comes in and that he would like to put in every possible intervention.

· Calling for a meeting with residents and/or residents’ family members.

· Checking LIC602A Physician’s Reports to ensure residents who bring their own materials like scissors are able and capable of having the items with them and ensure other residents who do not have the capability do not have direct access.

· Installation of camera in the common area.

· Ensuring items that can pose risks to residents are not readily accessible.
