On 7/3/2024 LPA K. Nguyen arrived unannounced for a continuation annual. LPA was greeted by care staff Ellen Jovero, Staff, LPA spoke with administrator over the phone to explained today visit and have verbal permission for Ellen to sign the report.
LPA review 6 clients files. 6 out of 6 have current physician report and T. B. on files. LPA reviewed 2 staff records and 2 of 2 have current first aid training and associated to the facility. LPA reviewed a sample of resident’s medications.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided via email.
