The inspector’s account

On 12/26/2025 at 2:00PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Required - 1 Year inspection. LPA met with staff, Joseph Rillera and explained the purpose of the visit. Administrator, Maria Fatima Lacsamana arrived 30 minutes later.

LPA toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, garage, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguisher was observed to be full. One week of nonperishable and 2-day of perishable food supplies were available. Hot water temperature was measured at 114 degrees F in the hallway bathroom sink. There were adequate lights in each room. First Aid kit is complete. Last fire drill was conducted on 12/22/2025.

LPA reviewed 4 clients and 3 staff files starting at 2:20PM. Client and staff files were complete. Staff are fingerprint cleared and associated to the facility. LPA reviewed client's P&I money with log. LPA reviewed a sample of client's medications during inspection.

At 4:30PM, LPA observed unlocked lighter and cigarettes in the kitchen drawers. Administrator locked up the items during inspection.

The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties.
