Facility Evaluation Report
On 7/29/2025 at 12:45PM, Licensing Program Analysts (LPAs) K. Nguyen and P. Manalo attempted to conduct a Case Management visit regarding an incident report received on 7/24/2025. Due to street construction LPAs was not allowed to enter (close off streets).
On 9/16/25 at 9:30 AM, Licensing Program Analyst (LPA) K. Nguyen to follow up on a previous visit Case Management dated 7/29/2025. LPA interviewed staffs and resident 1 (R1), and Resident 2 (R2). R1 stated there are no beg bugs or any bugs at the house.
LPA meet with Administrator (ADM) and explained the purpose of the visit.
LPA toured facility but not limited to residents bedroom, bathroom, kitchen, and backyard. During the toured LPA observed facility is clean and order free.
No deficiency observed during this visit.
Exit interview is conducted and a report is provided.
