While at the facility to deliver the findings for the complaint (Complaint Control # 15-AS-20250116091702), and during telephone conversation with Aidan Castrence, administrator (ADM), ADM informed LPA the licensee's intent to close the facility. LPA discussed the process of surrendering the license and facility closure. LPA also requested to submit a signed letter indicating the facility closure and copy of Board Resolution.
