The inspector’s account

On 7/09/2026 at 8:30 am, Licensing Program Analyst (LPA) K. Nguyen conducted an unannounced annual required inspection. LPA met with caregiver Theresa Abernathy. Theresa informed Administrator (AD) Glen Ocal via telephone, and LPA explained the purpose of the visit. AD arrived at the facility at 10:30 am.

LPA toured the facility, including but not limited to bedrooms, bathrooms, kitchen, common area, and backyard. The facility consists of 8 bedrooms, of which 7 are occupied by clients, and 1 is for staff. The facility consists of three (3) total restrooms, of which two (2) are used by clients and one (1) is used for staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water.

A comfortable temperature for clients is maintained at 71 degrees Fahrenheit. LPA observed that lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 113 degrees Fahrenheit. All toilets, handwashing, and bathing facilities are safe, sanitary, and in good working order. The supply of extra hygiene was available for clients. There is a minimum of a one-week supply of non-perishables and a 2-day supply of perishable food. Smoke detectors/carbon monoxide detectors were in operating condition during the visit. The fire extinguisher was last serviced on 6/10/2026. The last fire drill was conducted on 2/27/2026. The first aid kit was observed to be complete.

LPA reviewed two (2) staff files and eight (8) client files. LPA reviewed a sample of medication.

No deficiencies were cited during the visit.
