Facility Evaluation Report

On 3/24/2026 approximately at 10:45AM, Licensing Program Analyst (LPA) K. Nguyen conducted case management regrading an Unusual Incident Report (UIR) on 3/17/2026, LPA met with Administrator (ADM), Glen Ocal and informed him the reason for visit.

Facility reported on 3/18/26 client 1 (C1) reported missing from the facility. LPA follow up with ADM regarding the status of C1. LPA reviewed R1 files including but not limited to Admission Agreement, Appraisal/ Needs and Services Plan, Physician Report, Identification and Emergency Information.

Files’ review indicate that C1 is able to leave without assistance. When LPA conducted interview with Staff 1 (S1) stated that C1 is safe and admitted to Suttle Hospital San Mateo, and S1 received confirmation from hospital that C1 will be discharge back to the facility sometime tonight or tomorrow.

ADM stated that this is the first that with C1 behaviors. ADM agreed to give LPA a follow up when C1 is discharged. LPA requested documents from ADM to submit to CCLD by 4/7/26 including but not limited to updated needs and services plan, after visit discharge summary, and preventative plan that facility and care team for C1 to prevent this from happening.
