Facility Evaluation Report

On 5/19/2026 at 10:55 a.m., Licensing Program Analyst (LPA) K. Nguyen conducted an announced one-year annual inspection, met with Licensee Anthony Taylor, who holds a current and active administrator certificate, and explained the purpose of the visit. Fire clearance is approved only for ambulatory clients.

During the visit, LPA toured the facility, including 3 bedrooms, 2 bathrooms, a dining room, a common area, a kitchen, and a backyard. There is sufficient lighting around the facility. Client rooms are equipped with the proper furniture and lighting. Client's rooms have proper bedding and linens for clients' use. The kitchen was observed to be clean and in compliance. The bathroom is equipped with a non-slip mat and hygiene items. The living room is equipped with the proper furniture for the clients. Cleaning supplies and sharp objects are locked and inaccessible. Passageways and hallways are free of obstruction. Smoke detectors and Carbon Monoxide detectors are observed throughout the facility. The fire extinguisher was last inspected on 12/2/2025. The medication cabinet has a lock, and the first aid kit is complete. The hot water temperature is 118.4 degrees Fahrenheit. The fire drill was last conducted on 05/09/2026. The facility currently has a surety bond dated 2/5/26.

LPA reviewed 3 staff files; all 3 have TB and are associated with the facility.

An exit interview is conducted, and a copy of this report is provided.
