On 1/19/2026, at around 12:23 pm, LPA K. Nguyen arrived unannounced to conduct case management regarding an overdue license fee. Left rang the doorbell and was greeted by care staff, Marilou Kagaoan. LPA spoke with Licensee Corshema Taylor via phone and explained the purpose of the visit. The licensee was not available at the time of the visit. The licensee gave permission to Marilou to sign the report.
Licensee was able to show proof via text that Licensee paid the Annual Fee dated 1/29/2026 of $681.
An exit interview was conducted, and a copy of this report was provided.
