Facility Evaluation Report
On 5/05/2025 at 11:00 a.m., Licensing Program Analyst (LPA) K. Nguyen conducted an announced one-year required annual inspection, met with Licensee Corshema Taylor, and explained the purpose of the visit. Fire clearance is approved for ambulatory clients only.
During the visit, LPA toured the facility, including but not limited to 3 bedrooms, 2 bathrooms, dining room, common area, kitchen, and backyard. There is sufficient lighting around the facility. Client rooms are equipped with the proper furniture and lighting. Client's rooms have proper bedding and linens for the clients to use. The kitchen was observed to be clean and in compliance. The bathroom is equipped with a non-slip mat and hygiene items. The living room is equipped with the proper furniture for the clients. Cleaning supplies and sharp objects are locked and inaccessible. Passageways and hallways are free of obstruction. The fire extinguisher complies. Smoke detectors and Carbon Monoxide detectors are observed throughout the facility. The medication cabinet has a lock, and the first aid kit is complete. Hot water temperature is measured at 117 degrees Fahrenheit. Fire Drill was last conducted on 3/10/2025. The facility currently has a surety bond dated 01/09/2024.
LPA reviewed 3 staff files, 3 out of 3 have TB and are associated with the facility.
An exit interview is conducted, and a copy of this report is provided.
