Facility Evaluation Report
On 12/23/25, at 11:55 am, Licensing Program Analyst (LPA) Delmudo arrived unannounced to conduct an annual required inspection. LPA met with Eugene Elendu, licensee, and Maria Victoria 'Vickee' Andalis, administrator, and informed the purpose of this visit.
The LPA inspected the inside and outside of the facility, which included the kitchen, dining room, common areas, bedrooms, bathrooms, side yard and back yard. The central storage for medications was locked.
The facility has working smoke and carbon monoxide detectors. LPA checked and observed the fire extinguisher fully charge with tag showed serviced 5/06/25. Hot water temperature in one of the bathrooms was tested and measured at 109.5 degrees Fahrenheit.

Currently, the facility does not have residents.
No citations were issued during today's visit.
