Facility Evaluation Report

On this day, October 6, 2025, at 1:45 pm, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct inspection to ensure the health and safety of residents. LPA met with staff, Elmer Lopez and Elise Cooks. The staff stated that Roselyn 'Rose' Chand, administrator (ADM), was at the facility earlier and left about an hour ago. LPA called and spoke over the phone with ADM and informed the reason for visit. ADM stated she'll be back but authorized Charo Figueroa, staff, to be with LPA during inspection. ADM arrived at around 2:55 pm.

LPA conducted inspection and observed supplies of perishables and non-perishable food adequate. Facility has running water and electricity. LPA also conducted interviews.
