Facility Evaluation Report
On 2/6/2026 at 12:30 PM, Licensing Program Analyst (LPA), Y. Brown arrived unannounced conduct a case management health and safety check. LPA met with
LPA toured the facility with the Administrator including but not limited to residents' apartments, bathrooms, multiple activity rooms, kitchen, common area and courtyard. There are no bodies of water observed. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. The water temperature was measured at 115.1 degrees F.
Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. There are no imminent health/safety concerns on today's date.
No deficiencies cited during the Health and Safety visit.
