Facility Evaluation Report

On 07/20/2026 at 10:25 AM, Licensing Program Analysts (LPAs) P. Manalo and A. Christy arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Mia Moe, and explained the purpose of the visit. Administrator certificate is current.

LPAs toured the facility including inside and out but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 4 total bedrooms of which 2 bedrooms are occupied by the clients, 1 bedroom is occupied by staff, and 1 art room. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 127.7 and kitchen was 130.6 degrees Fahrenheit. All toilets, hand washing and bathing are sanitary and in operating condition. The supply of extra hygiene's was available for clients. There is a minimum of one week supply of nonperishable and 2-day perishables food supply.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 02/03/2026. Fire Drill was last posted on 05/15/2026. First aid kit was observe d to be complete.

At 11:22 AM, 3 of clients’ records were reviewed. At 11:52, 3 staff records were reviewed, 3 of 3 have current first aid training and 3 of 3 associated with the facility. LPA reviewed client's P&I money with log. LPA reviewed a sample of client's medications.
