Facility Evaluation Report

On today’s date, at 12:30 PM, Licensing Program Analyst (LPA) L. Fici and Licensing Program Manager (LPM) Y. Flores-Larios arrived unannounced to conduct a Component III presentation. LPA and LPM was greeted by administrator, Amelia Alfonso.

During the Component lll presentation, LPA provided applicant information on how to operate the facility within Title 22 regulatory compliance as well as how to avoid common problem areas. Applicant confirmed understanding of Title 22 regulations discussed and agreed to be in compliance.

Exit interview conducted with administrator and a copy of this report was provided to applicant.
