Facility Evaluation Report

On 03/02/2026 at 2:10 PM, Licensing Program Analyst (LPA) P.Manalo came unannounced to conduct a case management visit to follow up on the facility's staffing. LPA met with Administrator (ADM), Amelia Alfonso, and explained the purpose of the visit.

On 02/20/2026, Licensee, Yolanda Uy, and ADM disclosed that multiple staff turned in their resignation letter effective 02/22/2026. ADM stated that there will enough staffing through the weekend and was in the process of hiring more staff either from the other sister facilities or new employees. Additionally, ADM stated that there are 5 or 6 current staff members that are not resigning.

During the visit, LPA observe multiple staff members in the living room for a staff meeting. Interview with ADM revealed that S1 and S2 are scheduled for the 3 PM to 10 PM shift and S3 will be scheduled for 10pm to 6am shift later today.

LPA reviewed documents including but not limited to staff schedule for the week, clients' face sheet, clients' Individual Program Plan (IPP). A review of the clients' IPP revealed that 4 of 5 clients are independent with most of their Activities of Daily Living (ADLs), 1 of 5 clients need help with most of ADLs, and 1 of 5 require 24 hour supervision. Interview with staff revealed that the staff listed on the staff schedule are still working in the facility and are accurate.
