Facility Evaluation Report
On 07/02/2025 at 11:55 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Program Director, Vivian Largusa, and explained the purpose of the visit. Day program operates from 8:45 AM to 2:45 PM, Monday to Friday.
LPA toured facility including but not limited to three (3) classrooms, four (4) bathrooms, and a quiet room. Clients bring their own lunches and snacks. Emergency supplies, including water were observed. The hot water temperature in the client bathroom was measured at 116.8 degrees Fahrenheit. Medications are not handled or dispensed by this program. There are no bodies of water or fire safety hazards observed. Restrooms are maintained in safe and in sanitary operating condition. The program has 1 van used for client outings.
Emergency Drill was last conducted on 06/16/2025. Fire extinguishers throughout facility were last inspected 08/30/2024. Carbon monoxide detector was observed in operating condition. First aid kit was checked.
At 1:12 PM, LPA reviewed 4 participants’ files. At 1:31 PM, LPA reviewed 5 staff files.
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
