The inspector’s account

On 09/04/2025 at 11:50 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Program Director, Vivian Largusa, and explained the purpose of the visit.

LPA toured facility including but not limited to, resting room, library, multipurpose room, kitchen, bathrooms, office space, and the outside recreational area. Clients bring their own lunches and snacks. The hot water temperature in the client bathroom measured 114.8 degrees Fahrenheit. Restrooms are maintained in safe and in sanitary operating condition, and additional equipment for the physically handicapped was observed. Incontinent clients are kept clean and dry, and the facility is free of odors.

The program has 3 vans used for client outings. Van registration and insurance was reviewed during the visit. Carbon monoxide and smoke alarm were in operating condition. Emergency disaster drills was last conducted on 08/27/2025. Fire extinguishers were last inspected 01/28/2025. First aid kit was checked. LPA reviewed 6 client and 5 staff files for sampling. LPA reviewed one client’s medication.

THE FOLLOWING DEFICIENCIES WERE OBSERVED DURING VISIT:

At 12:45 PM, LPA observed cleaning disinfectant spray accessible to clients in the multipurpose room.

At 1:00 PM, LPA observed food and cleaning supply stored in the same storage room.

The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
