Facility Evaluation Report
On 04/02/2025 at 8:50 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Licensee, David Bloch and explained the purpose of the visit.
LPA toured facility including but not limited to, activity room, kitchen, bathroom, and the outside recreational area. Clients bring their own lunches and snacks. The hot water temperature in the client's bathroom was measured at 114.8 degrees Fahrenheit. Medications are not handled/dispensed by this program. There are no bodies of water observed. Restrooms are maintained in safe and in sanitary operating condition. The program has 1 van used for client outings. LPA reviewed client and staff files for sampling. First aid kit was checked.
