Facility Evaluation Report
On 12/29/2025 at 10:55 AM, Licensing Program Analyst (LPA) P. Manalo arrived announced to conduct a closure inspection visit. LPA met with Administrator, Robert T. Laguna and explained the purpose of the visit.
On September 3, 2025, Licensee sent LPA a 60 day notice of the closure letter on 09/05/2025. On 12/23/2025, LPA received an email from Licensee, Robert I. Laguna, that the last client moved out of the facility on 12/16/2025.
During the visit, LPA toured the facility including but not limited to bedrooms, bathrooms, common area, garage, and backyard area. LPA observed no clients present. ADM stated that the remaining two clients moved out of the facility on 11/01/2025 and 12/16/2025. Both clients safely relocated to another Adult Residential Facility (ARF) and Intermediate Care Facility for the Developmentally Disabled Habilitative (ICF/DD-H).
LPA obtained the original license and will send forfeiture letter at a later time.
