Facility Evaluation Report
On 05/08/2025, Licensing Program Analyst (LPA) P.Manalo conducted a case management visit while at the facility for another visit. LPA met with Caregiver, Rosario Mababa, and informed the reason for visit. The Administrator (ADM), Robert Laguna, was informed on the phone of the visit. ADM was not available to come and gave authorization for staff to sign the report.
During the visit, the ADM informed LPA on the phone that the facility will be closing. LPA and ADM discussed the facility closure regulation.
LPA is requesting to have the following document sent to CCLD:
Letter of facility closure to all the clients 60 days before the intended eviction.
Exit interview conducted and a copy of this report provided.
