Facility Evaluation Report
On 05/08/2025 at 1:05 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct Proof of Correction (POC) visit. LPA met with Caregiver, Rosario Mababa, and informed the reason for visit. The Administrator (ADM), Robert Laguna, was informed on the phone of the visit. ADM was not available to come and gave authorization for staff to sign the report.
On 03/26/2025, LPA conducted an Annual visit in which deficiencies were cited. The POC due dates for regulation 80087(a) and 80076(a)(7) was due on 04/25/2025.
LPA verified that the deficiencies have been cleared during the visit.
The exit interview was conducted and a copy of this report provided.
