Facility Evaluation Report
On 02/25/2026 at 4:00 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct a case management visit. LPA met with Administrator, Rosalinda Amacan, and explained the purpose of the visit.
While conducting complaint investigation 15-AS-20251120095534, S1 revealed that S1 helped C1 write down C1’s card information. S1 left the paper that contained the card information in C1’s room on top of C1’s desk. S1 stated that S1 was unsure who might’ve seen C1’s card information since any staff member has access to C1’s room daily.
S1 did not safeguard C1’s card information and left it in C1’s room.
The deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
