Facility Evaluation Report
On 01/02/2025 at 5:45 PM, Licensing Program Analysts (LPAs) P. Manalo and L. Fontanilla conducted an unannounced case management visit regarding a self reported alleged sexual abuse allegation that occured on 12/31/2024. Administrator self reported the incident to CCLD on 01/01/2025. LPAs met with Administrator, Tsion Haile Mogesse, and explained the purpose of the visit.
LPA interviewed C1 and Administrator. LPA will request the following documents such as C1's Physician's Report and IPP. The documents will be due on 01/21/2025.
The incident was reported to Fremont Police Department and is still under investigation. Once investigation is completed, LPAs will have to come back.
