Facility Evaluation Report
On 03/12/2026 at 3:00 PM, Licensing Program Analyst (LPA) P. Manalo conducted an unannounced case management visit regarding an incident report that was reported to CCLD on 02/05/2026. LPA met with Direct Care Staff, Fidel Castro, and explained the purpose of the visit. Administrator (ADM), Tsion Haile, gave verbal authorization for Castro to sign the report.
Incident Report indicated that C1 sustained a head injury and fracture from a fall and was discharged from the hospital on 02/04/2026. Incident Report also revealed that the toilet tank was broken where C1 fell. Interview with ADM revealed that the fall was unwitnessed and no one noticed that the toilet tank was leaking which caused C1 to fall. Interview with C1 and W1 revealed that C1 fell in the bathroom after visiting C1's roommate and C1's fall was caused by the toilet leaking.
The following deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiency by POC date may result in additional Civil Penalties.
