Facility Evaluation Report
On 03/12/2026 at 2:50 PM, Licensing Program Analyst (LPA) P.Manalo arrived unannounced to conduct a case management visit to verify the facility sketch. LPA met with Direct Care Staff, Fidel Castro, and explained the purpose of the visit. Administrator, Tsion Haile, gave verbal authorization for Castro to sign the report.
On 01/09/2026, LPA observed during the annual visit that C1 was staying in a room that was not approved for a nonambulatory client. ADM sent in the updated facility sketch and LIC200 to request for the fire department to do their inspection. On 02/13/2026, LPA received an approved Fire Safety Inspection Request (STD850) from the fire department.
During the visit, LPA verified that C1's room is the approved nonambulatory room per facility sketch and STD850. However, LPA observed that the facility sketch does not match the physical plant of the facility.
LPA will be requesting for the facility to send an updated facility sketch that corresponds to the physical plant of the facility by 03/19/2026.
