Facility Evaluation Report

On 01/09/2026 at 2:00 PM, Licensing Program Analyst (LPA) P. Manalo conducted an unannounced case management visit regarding an incident report that was reported to CCLD on 01/06/2026. LPA met with Administrator (ADM), Tsion Haile, and explained the purpose of the visit.

Incident report indicated that on 01/05/2026, Client 1 (C1) had a seizure at around 6:00 PM. Staff notified Administrator right away while the seizure was occurring and Administrator instructed staff to call 911. Paramedics came and transported C1 to the hospital for treatment. C1 returned to the facility later on that night.

During the visit, LPA interviewed Staff 1 (S1) and ADM. LPA reviewed physician report, appraisal needs and services, after visit summary, doctor's order for seizure medications and verified that the facility has the medications. Interview with ADM revealed that during the next staff meeting, ADM will communicate with the staff what the process should be like when there's an emergency happening.

LPA is requesting for an updated appraisal needs and services plan for C1 and for the facility to create a plan of action when there's an emergency with clients.
