Facility Evaluation Report
On 01/09/2026 at 8:50 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Tsion Haile, and explained the purpose of the visit. Administrator certificate is current.
LPA toured the facility including inside and out but not limited to 12 bedrooms, multiple bathrooms, gym, medication room, office, kitchen, common area and backyard. The facility consists of 12 total bedrooms which all bedrooms are occupied by the clients. There are no bodies of water. A comfortable temperature for clients is maintained at 70 degree Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 115.7 degree Fahrenheit. All toilets, hand washing and bathing are safe, sanitary and in operating condition. The supply of extra hygiene's was available for clients. There is a minimum of one week supply of nonperishable and 2-day perishables food supply.
Smoke detectors and carbon monoxide were in operating condition during visit. Fire alarm semi-annual was last conducted on December 2025. Fire extinguisher was last serviced on 10/31/2025. First aid kit was observed to be complete. Fire drill was last conducted on 12/20/2025.
At 10:20 AM, 6 of clients records were reviewed. At 11:17 AM, 4 staff records were reviewed and 4 of 4 have current first aid training and 3 of 4 associated with the facility. At 12:30 PM, LPA reviewed all the client's P&I money with log. At 1:00 PM, LPA reviewed a sample of client's medications.
