Facility Evaluation Report

On 6/25/2026 at 2:16pm , Licensing Program Analyst (LPA) Carol Fowler arrived unannounced to conduct a Complaint visit. LPA met with James S. Alegre, Administrator and explained the purpose of the visit.

While LPA Carol Fowler was conducting a complaint investigation (#15-AS-20260622095811) on 6/25/2026 during facility tour and interview LPA observed:

1. LPA and Administrator witnessed a mouse running into a water heater closet in the hallway.

2. LPA witnessed Administrator cashing a clients check and withholding funds.

The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
