Facility Evaluation Report
On 07/21/2026 at 10:30 am, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual 1-year required inspection. LPA met with Administrator Sheeva Bhutani and explained the purpose of the visit.
The administrator currently holds a certificate (#7011745740) that expires on 6/20/2027. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, office, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 75 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. LPA observed a minimum of one week supply of non-perishable and two (2) day of perishable foods.
Hot water temperature in the residents shared bathroom was measured at 115.0 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last purchased on 5/27/2026. First aid kit was observed to be complete. LPA reviewed three (3) staff and four (4) resident records. Emergency disaster plan was last reviewed on 5/6/2026. LPA observed a cabinet for medication and a drawer for sharps were locked and inaccessible to residents. LPA reviewed a sample of medication.
