Facility Evaluation Report

Licensing Program Analyst (LPA) Hiratsuka conducted this announced case management visit to confirm the facility is closed. This is in response to several meetings with Licensee Jennifer Esterline, informing Community Care Licensing Division (CCLD) and Alta California Regional Center (ACRC) on June 3, 2025, the program was closing. Licensee followed closure instructions by the ACRC and CCLD.

LPA arrived while a couple of clients were waiting to be picked up. LPA observed the inside of the building being cleaned up after the last client was picked up. LPA spoke to several staff members. Program Administrator Amber Dupuis was present. The final client left before 2:00pm.

LPA is going to close the facility file effective August 1, 2025. LPA is going to email a copy of this report to Licensee with a letter license forfeiture letter confirming the facility is closed.
