Facility Evaluation Report

On 6/17/26 at 2:00pm Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department on 6/3/26. The incident occurred on 6/2/26. LPA met with Administrator (A1) Angel Silva and stated purpose of visit.

The department conducted interviews and reviewed records. Based on internal findings the Staff did not push a resident while walking from one location to another within the facility. Per report the staff has received disciplinary action, including Administrative Leave and Staff, and will complete training that includes Conflict Resolution, Mandated Reporting, and Managing Stress. Because of the allegations that were unfounded, in the best interest of the facility, staff and residents, S1 is not performing duties in this location any longer at least on a temporary basis.

Exit interview conducted. A copy this report was provided to Administrator whose signature confirms receipt of this report.
