Facility Evaluation Report
On 07/21/26 Licensing Program Analyst arrived at the facility to complete an unannounced annual visit. LPA met with Program Manager, Celeste Estrada and Business Manager, Daisy Mendoza , explained reason for visit and was permitted entry into the facility. Executive Director, Timothy Joel Glick, arrived a short time later. LPA completed a tour of the facility inside and out. A health and safety check was completed on clients in care. Clients observed in activity areas.
The facility sits on a 3 acre parcel of property with a main activity building surrounded by smaller buildings. The foundation building located at 618 North Bridge and the Ice House Theater located at 410 East Race Street (both on the same parcel) are operated by other entities and not utilized by the program. Due to recent changes with the city's address reassignment, the facility is in the process of completing a new fire clearance.
Pathways and doors were clear and free from obstruction. Facility/Activity rooms were without odor, adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operate on a system. System last tested on 06/09/26. Fire extinguisher last serviced 07/10/26. Last fire drill conducted on 07/09/26. Sharps were located in locked cabinets. LPA observed sufficient seating under covered areas.
LPA requested the following documents to be submitted to CCL by 07/24/26: Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-D), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) and a current copy of liability insurance in order to update the facility file.
Exit interview was conducted with Program Manager and Business Manager. A copy of this report, provided to Administrator.
