Facility Evaluation Report

On 05/04/2023 at 1:30pm, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA Pascua met with staff member, Fernanda Soloranzo and explained the purpose of the visit. LPA asked SM Soloranzo to call the Facility Designated Administrator to inform them that CCL was present. LPA Pascua spoke with FDA Doris Woodruff and was informed that the LPA may move forward with the visit with SM Soloranzo. The purpose of the visit is in response to a case management visit that was conducted by the department on 12/07/2023 at 2:30pm.

On 12/07/2023, LPA Pascua arrived at the facility unannounced to complaint visit. Upon LPA arrival, it was learned that a resident was left unsupervised during a period of time at the facility. R1 answered the front door and informed the LPA that they were home alone and there were no staff members at the facility.

After further review, an immediate civil penalty of $1,000 was assessed for a violation of Section 80078(a). This civil penalty is due to a repeated violation within a 12 month period. The facility was vited on the violation above on 12/07/2022. The facility was informed that the civil penalty will continue to accrue $100 per day per violation until the deficiency is corrected.

An exit interview was conducted, a copy of the LIC 809, LIC809D, and appeals rights were given to the facility at the end of the visit.
