Facility Evaluation Report

On 11/20/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA was met by Donald Stewart and explained the purpose of the visit.

Current census 3. A brief interview with Donald Stewart was conducted.

LPA Pascua handed the Order to Licensee/Facility of Immediate Exclusion From Facility letter to Donald Stewart and explained that staff (S1) is excluded from any involvement in the facility effective immediately. Facility staff (S1) is to be removed from all shifts and disassociated from the facility in Guardian.

No citations were issued on today's date. A copy of this report was provided to the facility at the end of this visit.
