Facility Evaluation Report

Licensing Program Analyst (LPA) Noel Wolf Petersen arrived unannounced to conduct an annual inspection 11am, 4/17/26. LPA Wolf Petersen met with Jeevan Sandhu, Administrator, and explained the purpose of the visit. Delta Care is a one floor building with four bedrooms licensed to serve 4 ambulatory adults age 18 to 59, 2 of which may be non-ambulatory. No changes have been made to the facility floor plan.

two client present of the four clients that reside at the facility at the beginning of the visit. Two of the clients were at a day program. 2 Client files have signed admission agreements, recent 602 medical assessments, IPPs, and health assessments. 2 clients p+I was reviewed and found to be in accordance with balance sheet, 2 clients medications were reviewed and checked and found to be in accordance with the MARs.

All staff present during the visit and the remaining staff listed on the staff roster were found to have been finger print cleared and associated to the facility. 2 Staff files contained up to date cpr/first aid certifcation, annual and initial training, and health screenings. 2 staff interviews, 1 client interview.

LPA toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. facility is clean, traffic areas are well lit and unobstructed. Water temp is within 105-120*f range. Furniture and furnishings were sufficient to meet the needs of residents. Evacuation route gate is not able to swing freely. Administrator provided agreeable solutions to the the gate, either tightening a tension wire or otherwise. Fire extinguisher was dated 10/25, the smoke alarm is functional, first aid kit has all required items. Posters are in place as required.

Administrator records were checked including surety bond, present and up to date. No citations issued, a copy of the report was read and given to the administrator. Exit interview conducted.
