Facility Evaluation Report
At approximately 9:25 AM, Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by Tracy Skillman, Housemanager. Facility is an Adult Residential Facility (ARF) with four (4) clients in care. Client 1 (C1) and Client 2 (C2) were present during today's visit. LPA were informed that Client 3 (C3) and Client 4 (C4) were away in the community. Facility is serviced by vendor North Bay Regional Center (NBRC). Upon arrival, LPA was informed that one (1) of two (2) bathrooms was under renovation for updated appliances. LPA informed Housemanager that facility shall inform CCL before any construction takes place with a plan on how clients will be kept safe.
At approximately 9:40 AM, LPA initiated a tour of the facility with Housemanager and observed the following: Facility is a one-story home, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguisher was observed charged and was last serviced 1/2025. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperatures in client bathroom read within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, and paper products available for clients. Clients' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Housemanager informed LPA that Client 3 (C3) has addiction issues and his withdrawals have been making him destructive. Facility has been replacing items and cleaning room as needed. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two (2) days of perishable food and seven (7) days of non-perishable foods. Facility has internet available to clients in care, and the phone was tested an operational during today's inspection.
