The inspector’s account

On 06/12/2026 at approximately 11:00AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Adult Residential Facility (ARF) and was greeted by Sara Alegre, Lead Direct Support Staff (DSP). Administrator, Jenna Nichole Baluyot arrived shortly after. Upon arrival, LPA was informed that there were one (1) client in care and two (2) staff members on-site. The facility’s Fire Clearance has been approved for capacity of four (4) clients which three (3) can be ambulatory and one (1) non-ambulatory. The facility is a 1 story building with four (4) client bedrooms, one (1) staff room, two (2) bathrooms, and common spaces. Facility also has a shed in the backyard for extra storage.

At approximately 11:25AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 11:45 AM, the LPA and Lead DSP conducted a tour of the building and grounds, which were observed to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted. LPA observed activity supplies for client’s use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Toxins are stored in a locked housekeeping cabinet in the garage and under the kitchen sink. The facility has hot water signs at all faucets accessible to clients. Fire Extinguishers found to be last charged on 05/08/2026 at the time of visit. Carbon Monoxide and smoke detectors were present. There was enough lighting in all common areas, client rooms, and hallways. Medication is centrally stored and secure.

At approximately 12:20PM, LPA reviewed one (1) client record and found that the client has current care plan, signed admission agreements, and physician's report on file. Medication records are thorough and contained physician's order. LPA also reviewed two (2) staff records.
