The inspector’s account

On 10/15/2025 at approximately 9:25am, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct a Required 1 Year visit of this licensed Adult Residential Facility (ARF). LPA met with Staff Member, Louie Brian Abrantes. Licensee, Selene Cruz Astorga arrived at approximately 10:05AM. Facility is an Adult Residential Home that provides care and assistance for Adults with Disabilities. The facility has an approved fire clearance for a total capacity of 4 clients, 3 ambulatory and 1 non-ambulatory. Upon arrival, LPA was informed that there were currently 3 clients in care, with one client out for a doctor's appointment and another attending a day program. The facility is a single-story building featuring four client bedrooms, two bathrooms, one office, and common living areas.

At approximately 10:30AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 10:45 AM, the LPA and Licensee conducted a tour of the building and grounds, which were observed to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted. LPA observed activity supplies for resident use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Staff were in the process of cleaning up rooms at the time of this inspection. Toxins are stored inaccessible to the clients in the garage and under the kitchen sink. Water temperature measured 113.6 degrees F and 112.4 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents. During the inspection, the facility was repairing the hot water connections in bathroom number two by an external maintenance vendor.

Fire Extinguishers found to be last charged on 10/03/2025 at the time of visit.
