On February 12, 2026, Licensing Program Analyst (LPA) Ivan Avila, conducted a case management visit. LPA met with Administrator Krystle Love.
The department is following up on an Incident Report submitted by the facility on February 4, 2026 involving a resident.
LPA obtain further information regarding R1. During today's inspection LPA interviewed Administrator and collected documents.
No deficiencies cited during today's in inspection. Exit interview conducted and copy of report was provided.
