On December 11, 2025, Licensing Program Analyst (LPA) Ivan Avila, conducted a case management visit. LPA met with Administrator Mariel Hookham and explained the purpose of the visit.
LPA and Administrator discussed the importance of resident rights and allowing residents to discuss their concerns they might have with facility staff. LPA and Administrator discussed the importance of staying in compliance with Title 22 regulation.
No deficiencies cited during today's visit. Exit interview conducted and copy of the report was provided.
