On May 8 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct an unannounced case management. LPA met with Supervisor Glenna Miller and explained the purpose of the visit.
LPA Benson reviewed the staff files and interviewed the Supervisor, inspected the facility. Interview is attached in an 812.
Staff could not find the yearly staff training's. The back yard has weeds and six cats meowing on the porch.
No citations were issued per Title 22 Regulations.
An exit interview was conducted and copy of the report left at facility.
